If your Debit Card is ever lost or stolen, call us immediately at 502-633-4450 or after hours at 866-546-8273. If you already have a checking account with us, to order a new Debit Card please print the form below, fill in the information, drop it off at any of our convenient branch locations or Mail to: Citizens Union Bank P.O. Box 189 Shelbyville, KY 40066.

If you receive a phone call or text message claiming to be from Citizens Union Bank to re-set your debit card this is a phish attempt. Do not reply. Do not open any attachments. Planning an out of state of vacation? Remember to contact your local branch or the Customer Care Center at 1-866-633-4450 to let us know the dates and states through which you will be traveling. We will place a Travel Note on file to help prevent any delays with debit card transactions while you are out of town.

Need to reset your debit card PIN? Contact your local branch or the Customer Care Center at the number listed above for a temporary PIN reset. When contacting the Customer Care Center directly have your mother’s maiden name and driver’s license number available for security and identification purposes.
